Building a Form

The form builder is a visual tool for creating sample request forms. It shows a settings panel on the left and a live preview on the right so you can see changes as you make them.

How to create a form

  1. Go to Sample Requests > Forms in the sidebar.
  2. Click Create Form.
  3. Give the form a name (internal only, not shown to visitors).
  4. Configure the form settings in the left panel.
  5. Preview how the form looks in the right panel.
  6. Save the form.

Form settings

  • Fields: Choose which fields to include (name, email, company, phone, address, sample preferences, message).
  • CRM routing: If your CRM is connected, configure whether submissions should create or update contacts in your CRM.
  • Spam protection: Enable spam filtering settings.
  • Design theme: Select a visual theme for the public form (Classic, Bold, Minimal, or Catalog).

Form configuration

Forms are stored as JSON configuration, making them flexible and easy to version. The builder handles the JSON for you through its visual interface.
